Improving
student health and wellness in order to strengthen student
Academic
achievement, is a goal of the school nurse and health office services.
The school nurse
is available throughout the school day for student assistance in
case of illness,
injury or other health related concerns.
Health screening is conducted
throughout the
school year. Students, who need to
seethe nurse, are expected to
ask their
classroom teacher for permission to visit the nurse, except in the case
of an
emergency.
If illness or
injury prevents the student from completing the school day, parents will
be called so
that alternate arrangements can be made for the careof the student.
Please be sure that the Emergency Update Sheet,
on file in the nurse’s office is accurate.
It is essential that home and work phone
numbers for parents arecorrect. It
there are
other means of
contact such as cell phones and pagers, it is most helpful for the school
to have that
information as well. We also
request that parents designate an emergency
contact person
in case of a student emergency when parents are unavailable.
When the student
is unable to attend school because of illness, the parent must
notify the
main office624-7140. Messages may be left
at any time.
Immunization Requirements
All entering students must meet the following immunization
requirements before
registration:
1. Diphtheria – 3 separate specific dates are required, including
month/day/year.
2. Sabin Oral Polio (live) – 3 separate specific dates are required,
including
month/day/year.
3. If Salk or killed polio vaccine in used, 4 separate specific dates
are required,
all after 1/1/69.
4. Measles (Old Fashion Measles) – 2 doses of live measles vaccine, the
first
administered after 12 months of age and the second after 15 months of
age
(and more than 30 days after the first).
5. Mumps and Rubella (German Measles) – 1
live vaccine given after the first
birthday.
a.
Students having had the disease of Mumps are exempt from
the immun-
ization, provided serological evidence is verified by a medical
doctor.
b. After the age of eleven (11) immunization for Rubella (German
Measles) is
recommended but not required for females only. Males must have the
immunization or positive serology verified by a medical
doctor.
6.
Hepatitis B – the 3 dose series for children entering
grades K – 5 and 7 –10 during
the
school year 2003-2004. The
requirement extends to all students entering K –
11
in the year 2004-2005.
7. All students entering
from foreign countries in South or Central America,
Africa, Near, Middle or Far East need proof of freedom from active
Tuberculosis.
This proof may be in
the form of:
a.
A documented negative PPD administered in the U.S. in the past 12
months.
b. A
documented positive PPD with a documented chest x-ray, negative for TB,
administered within
the past 12 months in the U.S.
8. Chickenpox (Varicella) - For all
children born on or after January 1, 1998 AND are
entering
Kindergarten.
9. Chickenpox (Varicella) – For all
children born on or after 1/1/94 AND who enroll
in 6th grade at the beginning of 2005-06 school year.
a. Students who have had the
disease of chickenpox must have written
documentation from their Health care provider.
Immunization is required
prior to registration in Honeoye Falls-Lima Central
Schools.
Failure to provide proper proof of immunization will result in your
child's inability to attend
school on the first day.
Exemption from these requirements is grantedonly
for religious or
medical reasons, and both must be signed by the proper
authorities.Please contact the
school nurse for any questions.
Physical
Exams
New
York State law requires physical exams for all students who are new to the
district and
those
students who are entering kindergarten and first grades. The exam can be done as early
as
January of the year that the student enters these grades. The physician needs to complete the
Health
Appraisal form, which was sent to all kindergarteners (for first grade) and second graders
(for
third grade) in March. The Health
Appraisal form should be submitted to the nurse's office
by
October 1st. Please notify the
school nurse at 624-7145 if you would like the school
physician
to conduct the exam.
Medication
Policy
Please note: A Physician order as well as parent permission is now
required
for
all non-prescription as well as prescription medications that are required
during the school day or school related
activities.
Students, who require medication, either prescription or
non-prescription, during
school day or during school related activities, must meet the
following district
policies on a yearly basis:
1. Written
authorization from the prescribing physician which includes the
student's name, birthdate, medication, dose, route, time, date and prescriber's
signature, title, address and phone number must be on file in the nurse's
office.
2. Written authorization from the
parent must be on file in the nurse's office.
3. Prescription
medication must be in the original pharmacy container,
labeled with the student's name, medication dose, route, and time.
Non-prescription medication must be in the original container
with the
student's name attached.
4. All medication must be
delivered to the nurse's office by the parent.
Students are not permitted to have any medication, other than inhalers
or
Epi-Pens with prior medical approval, on their person or in their
lockers.
