Health Services at HF-L High School

Improving student health and wellness in order to strengthen student academic achievement is a goal of the school nurse and health office services.

The school nurse is available throughout the school day for student assistance in case of illness, injury or other health-related concerns. Health screening is conducted throughout the school year. Students, who need to see the nurse, are expected to request a pass from their classroom teacher except in the case of an emergency.

If illness or injury prevents the student from completing the school day, parents will be called so that alternate arrangements can be made for the care of the student.  

Please be sure to notify the central registrar at (585) 624-7123 of any phone or address changes for the Emergency Update Sheets.  This will provide accurate emergency contact information for your child.  It is essential that home and work phone numbers for parents are correct.  If there are other means of contact, such as cell phones and pagers, it is most helpful for the school to have that information as well.  We also request that parents designate an emergency contact person in case of a student emergency when parents are unavailable.

  • Health Appraisals:

    To all parents of students entering grades PreK or K, 1, 3, 5, 7, 9, and 11 in September 2018, there has been a change in the New York State Education Department physical exam requirements.

    The New York State Education Department requires physical examinations by a physician, nurse practitioner or physician’s assistant for students in certain grades. Beginning in September 2018 those grades are changing to grades PreK or K, 1, 3, 5, 7, 9, and 11.

    This means there will be some students who will require exams two years in a row if they had a physical required this year. This is not being imposed on you by the district. It is a new mandate by the State Education Department, and the district must follow the new regulations.

    The appraisals can be done as early as one year prior to the date that the student registers as a new student in the district or enters the required grade.  The Health Appraisal form must be submitted to the nurse by October 1st.

    Please notify the school nurse if you would like to schedule an exam with the school physician, free of charge.

     

    Mandated Screenings:

    New York State mandates the following periodic health screenings:

    11th grade: Visual acuity, Hearing Acuity, Scoliosis

    The School Nurse will conduct the screening if it is not documented on a current health appraisal form.

  • NOTE: As of June 13, 2019, there is no longer a religious exemption to the requirement that children be vaccinated against measles and other diseases to attend either:

    • public, private or parochial school (for students in pre-kindergarten through 12th grade), or
    • child day care settings.

    Refer to the NYS New Legislation on Immunization Requirement for detailed information.

    Immunization is required prior to registration in Honeoye Falls-Lima Central Schools. Failure to provide proper proof of immunization will result in your child’s inability to attend school on the first day. Please contact the school nurse for any questions. 

    All entering students must provide signed documentation from a physician or prior school health record showing that the following immunization requirements have been completed.

    A.  A signed CERTIFICATE OF IMMUNIZATION, or a signed statement from a physician, nurse practitioner or a physician's assistant*, or a NY State immunization registry record, or a signed out-of-state immunization registry, or laboratory reports showing unequivocal immunity to Varicella, Measles, Mumps, Rubella, Hepatitis B, or immunization records forwarded from previous school, all showing that a child has completed or has a return appointment and is in process of completing the following series of vaccines, as appropriate for age and grade: 

    1. DIPHTHERIA, TETANUS, PERTUSSIS, TDAP (GRADE 6) AND
    2. POLIO, AND
    3. MEASLES, MUMPS, RUBELLA, AND
    4. HEPATITIS B,  AND
    5. VARICELLA, AND
    6. MENINGOCOCCAL (1 DOSE GRADE 7,8,9; BOOSTER AT AGE 16)
    7. FOR PRESCHOOL ADD HAEMOPHILUS INFLUENZA TYPE B AND PNEUMOCOCCAL

    OR

    B.  IN-PROCESS:   A statement from the treating physician, nurse practitioner, or physician’s assistant* which shows the immunizations are in process, the child has received at least one dose of any age/grade appropriate vaccine, (or alternative proof as outline in section [A] above) AND has a return appointment to continue the immunization process AND is not outside the minimal interval for re-vaccination, such as a missed appointment,

    OR

    C.  MEDICAL EXEMPTION:  Written evidence from the treating physician, nurse practitioner, or physician's assistant* licensed to practice medicine in this state stating that there is a medical contraindication for one or more specified required immunizations, citing the specific vaccine, the specific duration of exemption, and the specific medical contraindication based on the Centers for Disease Control published standards AND as approved by the school medical director (Note: NYSED forms for medical exemptions are available from the school nurse),

    OR

    D.  HOMELESSNESS:   When a family has a state of homelessness, entry is not denied for lack of immunizations or proof of immunizations; however, the school nurse will work with the family to get immunizations done for the child's protection.

     
    *NOTE: TUBERCULOSIS (TB):

    A TB test done within 12 months prior to school registration is strongly recommended for any student entering the district who has had extended (greater than six weeks) domicile or travel in or from any country except USA, Canada, or Western Europe.

    Refer to the NYS Immunization Requirements Chart for required vaccines.

  • The NYS Education Department requires new medical orders every academic year if your child has a physician’s order for: 

    • Medication(s)- prescription and over the counter (students can not take over the counter medications such as Ibuprofen, Tylenol, and/ or cough drops without an order from and MD)
    • A life threatening or serious medical condition that requires an Emergency Care Plan (ECP); or Adaptive physical education

    Please keep a copy for yourself and drop off a copy of your healthcare provider’s orders by the first day of school.

  • A Physician’s order and parent permission are required for all non-prescription as well as prescription medications that are required during the school day or school-related activities.

    Students, who require medication, either prescription or non-prescription, during the school day or during school-related activities, must meet the following district policies on a yearly basis:

    1. Written authorization from the prescribing physician which includes the student’s name, birth date, medication, dose, route, time, date and prescriber’s signature, title, address and phone number must be on file in the nurse’s office.
    2. Written authorization from the parent must be on file in the nurse’s office.
    3. Prescription medication must be in the original pharmacy container, labeled with the student’s name, medication dose, route, and time. Non-prescription medication must be in the original container with the student’s name attached.  
    4. All medication must be delivered to the nurse’s office by the parent. Students are not permitted to carry any medication on the bus. 
    5. Medication will be stored in the nurse’s office.
    6. Under special circumstances, self-administration may be allowed. Please contact the school nurse for further information.

    All medication not picked up by parents at the end of the school year will be discarded during the summer. 

  • The Centers for Disease Control (CDC) has indicated that influenza is widespread at this time of year. While a shift in the strain of flu this year has resulted in more vaccinated people than usual developing flu, vaccination still has been shown to lessen the risk for serious illness and hospitalization should one develop the flu. Each year otherwise healthy children who are not vaccinated die from influenza, over 50, in fact, this year already. Very young children and those with chronic illness are at highest risk. Vaccinating others who care for vulnerable children can lessen the risk of illness. Of importance, especially in the very young, the very old, and people with chronic medical conditions, if the flu strikes it is best to check with your health care provider early to assess whether anti-viral medications will be useful to reduce the risk of severe complications from the flu.
     
    This is also to remind you of the continued availability of the 2019-20 influenza vaccine that you can get through your health care provider or at flu clinics and pharmacies this year. As you are aware, influenza remains an annual illness that is generally preventable by a yearly vaccination. One year's vaccine does not give lasting protection for the coming year's flu viruses.
     
    Flu season generally runs between November and May each year, but most cases of flu illness occur in January, February, and less so March.  It takes about 2 weeks to develop protection following the flu vaccine. The recommendation from the CDC is that all people who do not have allergies to components of the vaccine get the vaccine early in the fall when it becomes available, but if you missed it then, please consider it now.
     
    The district wants to encourage you also to have plans in place should you have a need for extended childcare in the event your child is ill and cannot attend school. As always, please remind your children of the importance of frequent and good hand washing, good respiratory hygiene to cover coughs and sneezes and prompt disposal of used tissues, and of not sharing personal items with others.  Please keep ill children home from school until they are well enough to attend a full day of school without fever-reducing medicines, and do remember to call in your child’s absence with the reason for absence.   • Athletes must complete registration online. The registration portal for the JV and Varsity upcoming season will be open as follows: 

    • OPENS:         October 12, 2019
    • CLOSES:        November 5, 2019
    • RE-OPENS:    November 11, 2019


    Registrations entered in Family ID on season start date, November 11, 2019 when portal re-opens, will be considered  "late" and as such will be subject to 24 hour minimum processing time for clearance.

    If there is a volume of late registration, or forms are missing information and/or not properly filled out, clearance time may be extended.

    It is our hope that the convenience of online registration will minimize or eliminate that issue for your athlete.
